Child Dedication Interest Form (M)

Thank you for expressing interest in dedicating your child at Keystone MV. We will use this form as a guide to help your family with the best next steps regarding our Child Dedication Class and Dedication Day.

Child Dedications happen in the spring and fall seasons at Keystone MV, generally on a Saturday morning. Prior to the Dedication Day, you will be required to participate in our Child Dedication Class (generally on a Sunday morning during the 11:30AM Worship Service). This class takes place 4-6 weeks prior to the Dedication Day. If your family has not been attending Keystone for over 3 months, please consider waiting to fill this form out until then.
